The Girl in the Pullman plot

A neurologist mainly specializes in deceiving his patients. For example, he uses the 'electrical cabinet' to 'treat' people. When his ex-wife shows up at his practice, he runs away. He meets his fiancé and her mother in a hotel for lunch, but then his ex-wife shows up again. And so it goes on, she chases after him and flirts with him, the doctor is at his wits' end. He decides to escape by train with his fiancée to get married elsewhere quickly. But he doesn't know that his previous marriage hasn't even been officially dissolved yet...
